OSF Holy Family offering sports physicals for $10

OSF Holy Family Healthcare is offering sports physicals for just $10 for all area athletes on Thursday. No appointments are needed as it is walk-in only. Pre-Concussion Baseline Screening using the ImPACT™ Computerized System will be available at no extra cost. Athletes MUST bring an IHSA Form completed and signed by a parent/guardian. You can download the form from the osfhealthcare.org website. For athletes that wear contacts or glasses – you are required to bring them to the physical. No credit/debit cards accepted. Exact change is appreciated.
