Adult Transition Independent Living Advocate Carrie Bewley Says Stone Hayes Remains Open by Appointment Only

Share

Stone Hayes Center for Independent Living continues to be open by appointment only for assistance through any of their offered services, including adult transitioning. Carrie Bewley, Independent Living Adult Transition Advocate has more:

“The adult transition, they get a referral from nursing homes normally. They have to be in the nursing home in order to be transitioned out. I go visit with them numerous times, ask them a series of questions to make sure they can live on their own safely. I follow them for 90 days, which means I just kind of keep in touch with them, make sure they have the resources they need, and make sure they are doing well. If they need any tech equipment to help them live independently, then I make sure they have that. Then we personally buy a month worth of groceries, we pay their rent, we pay their deposit for the first month, then they get to live on their own and have the freedom that they need.”

Throughout the month of May, if you purchase a reusable ‘My Heart Bag’ at Hy-Vee in Galesburg, a portion of the proceeds will benefit Stones Hayes Center.
