Yellow medication forms are now available at Strom Center for individuals to fill out and return for open enrollment, October 15th through December 7th. Executive Director Carol McCrery says there is a new form this year:
“We do have a little bit of a tweak on our form this year. We have added a couple of other columns on that. We have dosage amount and then if the medication is in tablet or capsule form, please note that. There can be a tremendous difference in prices between a table and capsule. Once they complete those forms, they can come in and either hand them to Lisa, who is at the front desk, or take them to the Oquawka site, or put them in the box right there, no one can see them, just fold them in half and there is a slot there to put them in the box in the front entry way.”
Three Medicare informational meetings will be held in the first part of October, one in Henderson County and two in Warren County, with more details to be announced later.
