Stone-Hayes Center Strives to Empower Independence Through Home MOD Program

With the goal of keeping individuals safely in their homes as long as possible, Stone-Hayes Center for Independent Living is able to do home modifications through a new program, explains Community Reintegration Program Coordinator Sarah Nelson:

“We have the Home MOD Program where we can do ramps, chair lifts; we can do anything interior, adjusting door frames so they are wider, bathroom remodels, you just have to reside in Illinois. It has to be a documented disability and meet the maximum income requirements. You can call us at 309-344-1306 and talk to Lonnie Baker, who is our Program Director. So, anything that you need to keep you at home longer, that is our goal.”

Stone-Hayes Center for Independent Living serves individuals in Warren, Henderson, and Knox counties.
