Deputies Respond to Head-On Collision in Fulton County

On 12/23/24 at approximately 7:46pm, Fulton County Deputies were dispatched to a two-vehicle head-on collision located on U.S. Rt. 136 between Main Gate Rd. and Ayrshire Rd. Table Grove Fire and Fulton County EMS were also dispatched to the scene. On arrival, deputies were notified that one driver, Adam Stevenson, 48 of Astoria, IL was deceased and later positively identified by Fulton Coroner Austin Rhodes. The driver of the second vehicle, Levi Palm, 21 of Table Grove, IL was treated by Table Grove Fire and Fulton County EMS personnel and was taken by ambulance to the McDonough District Hospital.

Preliminary investigation of the incident revealed that Stevenson was traveling eastbound when his 2006 Honda Ridgeline crossed the center line and struck Palm’s 2004 Dodge Ram in the westbound lane. Deputies and rescue personnel closed U.S. Rt. 136 for approximately six hours while the Illinois State Police conducted an accident reconstruction investigation, and the vehicles were removed. Assisting at the scene were members of the Fulton County Sheriff’s Office, Table Grove Fire Department, Ipava Fire Department, Fulton County EMS, Fulton County Coroner’s Office, and the Illinois State Police.
