Summer is quickly winding down and it will be time to start school before we know it. When you start making your back-to-school to-do lists, do not forget to add appointments for school physicals and immunizations. The Knox County Health Department and the Knox Community Health Center are here to provide you with a one stop shop for all immunization and physical requirements.
The Knox County Health Department, located at 1361 W. Fremont St., Galesburg, offers all vaccinations needed to meet the Illinois School Immunization Requirements. If your child is behind on immunizations, our providers can help get them back on track. The Health Department accepts Medicaid, insurance, cash, checks, and credit cards. KCHD also offers the Vaccines for Children (VFC) program, which provides free or low-cost vaccines to those eligible. Any child from birth to 18 can qualify if they are eligible for Medicaid, uninsured, underinsured, or American Indian or Alaskan Native.
The Knox Community Health Center offers school and sports physicals by appointment Monday through Friday 7:00am-5:00pm. Families who do not have a primary care physician for their kids can get connected with providers at the Knox Community Health Center who can provide children with annual physicals. If your child is entering:
- Pre-School/Head Start
- Kindergarten
- 6th Grade
- 9th Grade

As families start compiling their back to school to do lists, the Knox County Health Department and Knox Community Health Center can cross vaccinations and physicals off that list.
