Screening requirements include the child being 5 years old on or before September 1st and submitting a government issued birth certificate. Children must screen in their home building. Dates are May 7th at United North and May 8th at United West from 3:30 to 5 pm by appointment. Call United West at 309-734-8513 or United North at 309-482-3332.
