Each year, the Monmouth Area Chamber of Commerce recognizes an individual for their outstanding leadership in improving the quality of life in the Monmouth area community. The nominee does not need to be a Chamber member; consideration will be given based on the nominee’s contributions to his or her business profession and the Monmouth Community.
If you would like to honor an individual for their contributions, please mail the following completed form to the Monmouth Area Chamber office at 89 Public Square, Monmouth IL 61462 OR email the form to kelli@monmouthilchamber.com.
