Eagle View Offering Sports Physicals May 4th at MRHS

Eagle View Community Health System will be offering Sports Physicals at Monmouth-Roseville High School on Wednesday, May 4th from Noon – 4:30 pm Cost: $10 – if you will have a student next year involved in any sport activity, they must have a physical. These sports physicals are good for 1 year. These sport physicals do NOT replace the annual exam required for: NEW, Prek, K, 6th or 9th grade students. Must bring the form linked below with the front filled out with parent & student signatures on the front & back. **Students can pick up a copy from Coach Adolphson as well.
