What are Health Centers, also known as Federally Qualified Health Centers (FQHCs)? Community Health Centers are located in, and serve, high-need communities; Knox County is designated as a Healthcare Provider Shortage Area for primary care, dental and mental health services. However, the Knox Community Health Center offers comprehensive health care services as well as supportive services (education, translation, and transportation assistance) that promote access to health care for all members of the community, regardless of your ability to pay.

The mission of the Knox Community Health Center is to provide quality and compassionate health care services to all in need, regardless of their ability to pay. For patients that are uninsured or underinsured, we offer a Sliding Fee Discount Program which can provide discounts on medical, dental, and behavioral health services to individuals based on income and household size. The Community Health Center also accepts most insurance plans including Medicare, Medicaid (including the Managed Care Organizations) and private insurance. Individuals with or without insurance are encouraged to apply for the Sliding Fee Scale Program because if qualified, the Community Health Center will file with your insurance plan; however, you will never be responsible for more than you would pay under your Sliding Fee Scale Program approved Plan.

For more information or to schedule an appointment with one of our providers, please contact the Knox Community Health Center at 309-344-2225, Monday thru Friday 7:00am-5:00pm. The center is located at 1361 W. Fremont Street, Galesburg, IL. We want to be your medical home!
