Care Management in healthcare is a proactive approach designed to enhance the coordination of patient care, reduce the risk of complications, and work to prevent hospital admissions or readmissions. Registered Nurse and Chief Population Health Officer at Solvera Health, Robin Henry, informs once chronic conditions are identified, working with a Care Coach could be a patient’s ideal option:

“It can start with even hypertension. We have patients that we serve with Congestive Heart Failure, Chronic Obstruction Pulmonary Disease; it is these disease criteria that really are not going to go away, but maybe medication, lifestyle modifications, diet, exercise, can help alleviate it. What we are coming to see is that as healthcare is becoming smarter every day, our patient population is able to live longer and maintain these chronic illnesses while being able to do it in their home or wherever they call home.”

Care Coaches serve as a single point of contact between your provider and your specific healthcare needs by offering monthly phone consultations, remote monitoring of symptoms, health education, support, and community resource suggestions.
