Children must be at least 3 years old by the screening date. Please bring the child’s certified birth certificate, proof of household income, and most up to date physical. Screening will be held at Lincoln Early Childhood School in Monmouth from 9 to 11:30 am and 1 to 3 pm. Call Claudia at 309-734-2222 to schedule a screening appointment.
