Sports physical day at the MDH Monmouth Clinic is Thursday, July 11th from 9 a.m.-12 p.m. or 1-5 p.m. Cost is $10 and payable by cash or check. Walk-in appointments only for the sports physicals. Students should print off the IHSA sports physical form from your respective school’s website or the IHSA website.
