United JR/SR High Athletes Sports Physicals

OSF is offering Sports Physical for United Junior and Senior High School Athletes at the Junior/Senior High School Building. To schedule for 6th graders, call the High School Office at 309-734-9411, 7th and 8th graders can sign up with Margo, and high schoolers can sign up with Jessica. Cost is $10 and a form must be signed by a parent. Physical will be done a first come first serve basis. Intermediate students will ride the bus to the junior high and then need to be picked up by a parent.
