The Family Case Management program at the Warren County Health Department has shifted to Better Birth Outcomes. Administrator Jenna Link reports the program has similar goals in providing pregnant moms and infants with local resources:

“Similar in services and desired goals, but really it walks pregnant moms and moms with newborns up to the age of six months that are identified that maybe having a higher risk pregnancy or some other underlying putting them at risk. They can be a part of the program, and our nurses walk through with them and make sure they are connecting with their OBGYN and getting those visits in and follow up visits after baby is born with a pediatrician. It is just connecting them with the right resources to help with better outcomes for both mom and baby.”

If you are expecting a baby or parenting a newborn, reach out to the Warren County Health Department at 309-734-1314 to see if your family is eligible for the Better Birth Outcomes Comprehensive Program.
