Western Illinois Home Health Care Announces In-Home Primary Care Division

Western Illinois Home Health Care, an independent provider of in-home care, announced that it has hired Mandy Kubis, APRN, as its first provider to bring Direct Primary Care, transitional support, and specialty services to families in West Central Illinois. The new company, Western Illinois Home Provider Care, Inc. (WIHPC) will be an affiliate of Western Illinois Home Health Care.

Kubis has a background in oncology, med-surg, and emergency room care, but found her calling in home care, where she has dedicated the last eight years of her career. Her passion for serving patients in the comfort of their homes led her to become Wound Care Certified (WCC) in 2019 and a Board-Certified Family Nurse Practitioner (FNP-BC) in 2023, showcasing her commitment to advancing her skills for the benefit of her patients. Kubis was born and raised in Macomb, IL and continues to call this community home, residing with her husband, Jason and three sons, Riley, Mason, and Jaxon. Beyond her immediate family, Mandy is also a stepmother to two stepsons, Jayce and Asher, and a stepdaughter, Haley.

“I’m excited to bring this model to our local community. With my prior home care experience, Western Illinois Home Health Care was a natural fit to bring this unique service to our region. It truly is a special privilege to see patients in their own home,” Kubis said.

In addition to providing care and oversight to seniors and families, WIHPC will partner with other health care organizations to assist frail and medically-complex patients transition from local acute and post-acute facilities back into their home to increase patient satisfaction and overall outcomes.

WIHPC will provide care for residents living in Fulton, Hancock, Henderson, Knox, McDonough, Schuyler, and Warren Counties.
