Authorities are identifying the woman killed in a crash on U.S. Route 136 in Fulton County. Melissa Phelps died following the Monday’s accident. State police say a passenger car and commercial vehicle collided where Route 136 intersects with Illinois Route 78, just west of Havana. The crash is still under investigation.
